Register as an out of area patient
We now register patients outside of our catchment area.
New rules mean that we can now register patients who do not live within our practice area.
If we accept you onto our list as an “out of area” patient and it becomes apparent it is no longer clinically appropriate or practical we will discuss this with you to see if it would be more appropriate for you to be registered with a GP practice closer to your home.
What happens if I’m not well enough to come to the surgery?
You will have access to all the services we provide to patients in our area, however, we will not be able to provide you with home visits.
If you are too ill for the practice to help you over the phone, call NHS 111. The NHS 111 service will be able to tell you about access to local services or, where necessary, arrange a home visit.
In order to register you must complete both of these forms. Once completed we will consider whether you are suitable to be registered as an out of area patient.
