Please provide a mobile telephone number wherever possible. By doing so, you will be automatically enrolled to our text reminder service. We will use this for: contacting you about your health when unable to contact you at home / texting your appointment reminders / responding to your queries / advising when your prescriptions/paperwork is ready / advising you if appointments are cancelled.

Organ donation in England has moved to an 'opt out' system. This means that all adults in England will be considered to have agreed to be an organ donor when they die unless they have recorded a decision not to donate or are in one of the excluded groups.

You still have a choice about whether or not you wish to become a donor. If you wish to know more on the law and Opt Out from the register, please visit NHS Organ Donation.

You can self-register on the NHS Blood Donor Register, if you wish to become a blood donor. Please visit the website to register.

If you have medications please ensure you book a routine appointment so your repeat prescription(s) can be set up BEFORE they are due - there is a lead time to routine appointments being available.

Please specify name and location of chemist, as from 17th June 2014 all prescriptions will be sent electronically to your nominated chemist.

We have a patient participation group who meet with the practice on a quarterly basis. This group provides an effective way of patients and the surgery working together to improve services and to promote health and improved quality of care.

If you are interested in joining the group, please complete the Patient Participation Group Registration Form.

In compliance with the Race Relations (Amendment) Act 2000 and its Race Equality Scheme any new patient registrations are requested to complete this form.

When registering with a new GP in the UK you are automatically enrolled for an NHS electronic summary care record. A summary care record contains information about the medicines you take and any allergies you suffer from.

Having this information stored in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP Practice is closed.

It is easy to opt out of having a summary care record, simply ask at reception for an opt out form or visit: www.nhscarerecords.nhs.uk/optout

NHS App:

We are asking all new patients who register with us to download the NHS App to enable you to:

• Renew repeat prescriptions
• Book and manage appointments
• Securely access your medical records
• Check your symptoms, get instant advice, and more

It will be your responsibility to keep your login details and password safe and secure. If you know or suspect that your record has been accessed by someone that you have not agreed should see it, then you should change your password immediately.

If you can’t do this for some reason, we recommend that you contact the practice so that they can remove online access until you are able to reset your password.

If you print out any information from your record, it is also your responsibility to keep this secure. If you are at all worried about keeping printed copies safe, we recommend that you do not make copies at all.

The information that you can see online may be misleading if you rely on it alone to complete insurance, employment or legal reports or forms.

Be careful that nobody can see your records on screen when you are using Patient Online and be especially careful if you use a public computer to shut down the browser and switch off the computer after you have finished.

There are a few things you need to think about before registering for online records. On very rare occasions:

• Your GP may not think it in your best interest for you to look at your GP records online. If this happens, your GP will discuss their reasons with you. It is up to your GP to decide if you should be allowed access to your online records.
• You may see your test results before your doctor has spoken to you about them. This may be when you cannot contact your surgery, or when your surgery is closed. This means you will need to wait until an appointment is available to talk to your doctor.
• Information in your medical records might need correcting. If you find something you think is not correct, you should contact your surgery. The staff will be able to answer your questions and set things right when needed. Please bear in mind that you cannot change the record yourself.
• There may be information in your medical records that you did not know was there or that you had forgotten about, such as an illness or an upsetting incident. If you see anything you did not know about that worries you, please speak to your surgery and they will discuss this with you.
• If you see someone else’s information in your record, please log out immediately and let your surgery know as soon as possible.
• If you have questions about any of the above points, please talk to your surgery and they will be able to advise you further

For patients aged 16 years and over

Proxy access allows parents, family members and carers to access health services on behalf of other people. For example, children, dependants you care for, and relatives. Depending on the level of access you request, you can switch profiles to act on behalf of another person and:

• book an appointment
• order a repeat prescription
• view their GP health record, where appropriate
