Forms and letters are based on facts recorded in your medical records. The GP will not provide an opinion. If is is appropriate, you may be offered a copy of your medical records instead (whether in full or a date range) This is free of charge. SAR’s (Subject Access Request) and requests for full copy records take up to one calendar month. Payment is required in advance. We are unable to take cash payments. Payment can be made by Contactless card at Reception/Cheque (made payable to Castle Medical Centre)/Using your Online banking app. |
